DALEY DENTAL DISCOUNT (D3) MEMBERSHIP PLAN

We are now offering the Daley Dental Discount Membership Plan, the D3 Plan, our in-house dental discount membership! This membership is for patients not fortunate enough to have traditional dental "insurance". There are 2 convenient membership plans to choose from based on your dental needs.


Our membership plans have no yearly maximum and no deductible. Each plan includes

  • 2 routine oral evaluations
  • 2 healthy mouth cleanings
  • 2 oral cancer examinations
  • 2 sleep apnea screenings
  • 2 TMJ screening, and routine x-rays


Participants will also receive a 15% discount on most* dental services here at Daley Dental. The D3 membership plan can be used on fillings, crowns, root canals, and much more. There is no paperwork, no card to carry and no waiting for a pre-determination of benefits. We are taking the guess work and hassle out of traditional insurance processing. Children’s Plan (13 and Under)    

  $259 per year

The Children’s Plan Includes:

  • 2 Routine Oral Exams
  • 2 Healthy Mouth Cleanings
  • Routine X-rays
  • 1 Fluoride Treatment
  • Oral Cancer Screening
  • 15% off dental services*
Adult Plan      

$295 per year

The Adult Plan Includes:

  • 2 Routine Oral Exams
  • 2 Oral Cancer Screenings
  • 2 Sleep Apnea Screenings
  • 2 TMJ Screenings
  • 2 Healthy Mouth Cleanings
  • Routine X-rays
  • Oral Cancer Screening
  • 15% off dental services*

* This plan does not apply to dental product purchases, Invisalign, Healthy Start, Sleep Appliances, Botox, Fillers, Veneers, antibiotic delivery, or denture repairs.

** All Plans are Non-Refundable and Non-Transferrable and may not be used in conjunction with health care specific credit cards such as Care Credit, etc. 

*** Payment is due at the time of service. If payment for discounted services is not made at the time such services are rendered, such services shall not be eligible for any discount hereunder, and the full undiscounted price shall be due and payable. 

**** Membership is Valid for 12 Months from Date of Purchase.

Dental Insurance


We want you to get the most out of your budget, so we work with your insurance provider to ensure you get the best coverage available to you.

We are a Preferred Provider (PPO) for many dental insurance companies.  If you need assistance, or have questions about your insurance policies or claims, our staff is knowledgeable and always available to help you.
